I remember seeing an FAA doc that stated to use"MEDEVAC" when with patient and your N number when no patient (although most ATC's are OK with your medical call sign with title and number(LifeGuard11, AirSTAT5, MedLife, AirEvac75,LifeSaver, Childrens, etc))
