Is that helicopter designed with a galley, and with benches and rowing appliances that are manned by the medcrew?
When patients are absent, the medcrew will be assigned crewmember positions there, and the output of their rowing efforts will be transmitted to a portion of the main transmission to assist in keeping the helicopter flying. No sprag clutch, so they have to keep rowing during autorotation.
