But the pilot employed by the certificate holder does not have to release his or her personal information to the outlying agency [hospital in this case].  That was the original post.

As far as being an agent, that is required for someone which may or may not be employed by a certificate holder and lives out of the country.  The agent for service status or requirement is the local USA mailing address for all legal documents and contact information to receive FAA services.

Generally, the law firm is the external entity who represents the certificate holder and is listed as the "Agent for Service."  There may be others as authorized.
