Miami will have you cross mid-field at 2000’ AGL. Some Class D’s are having you cross at 1500’ AGL. (In central and south Florida) - If you are flying under “MEDEVAC” call-sign then state “Request” on initial radio contact then request to cross mid-field at one of those altitudes. I haven’t had any denials thus far. Another option is to contact the tower chief at that facility to establish an altitude that will accommodate both parties. Most facilities will go out of their way to help you out.
