leaving wrenches on xmsn decks after major maintenance, being told, "that's what the customer wants", decline a flight due to duty time but being told you can get an extension after your explain why you will go over but they have a spreadsheet showing options A,B, & or C instructing you to take the flight, not putting chip detectors back in after inspections, attempt to decline an LZ cause you feel it's inappropriate but being told, "we land there all the time", forgetting to install essential parts on m/r, giving monetary incentives to AM for in service rates, calling after declining a flight due to thunderstorms, (yet another program took flight)  ignoring AIDMORs documenting said issues. those a only a few I can think of off the top of my head
