"On occasion" is the point here. Acadian flew two pilots with one medic. PTN/MH will fly two pilots and two med crew in the 135. I'm sure they'll figure it out, but it does sound challenging at times.

I too have "occasionally" flown 5 or 6 people in a 135 or 109 or 206L4. But the point was that EVERY time they will have 5 people in there on a patient flight. 

No reason for you to puff out your chest and attack my experience when you just aren't understanding the point.
