Used to be Lifeguard until 2012. Then went ICAO and it is now only Medevac. The pilot in question is getting the idea to use hospital from foreflight ICAO flight plans.

If you read the Advisory Circular, there is only one approved callsign prefix for HAA, Medevac. Do not use Life guard, do not use Hospital or any other. Do not file IFR with the prefix L either.

That all went away in 2012. Lastly, you do not need a LOA or MOU to use Medevac prefixes. It's recognized nationall. Read the Advisory Circular. I don't have time to look it up now. It changed the same time ADS-B came out.
