Message Pilots have a duty to refuse to fly if they have any doubt as to their being able to safely complete it because of a health issue of any sort. Requiring a note attesting to your illness discourages pilots from best safety practice.
If and when an incident, accident happens and the pilot was sick and 'discouraged' from sick days in any way, lawsuits will fly- and be successful.
