Short term for us starts 14 days after the "claim" is made.  Most people will use their sick/vacaction/PTO time to fill in that gap.  

The 60% is reportedly not taxed but if you have premiums that are paid out of your paycheck normally you will have to cover those out of pocket to keep your coverages.  

Though it does sometimes happen and require clarification, your claim should continue until you get your medical back.
