Depends on the state.  I've seen nurse/nurse, medic/medic, and even the very rare doctor--comes with varous restrictions, too.  Doctor from traditional programs that want to justify having their pretty matching flight suit by flying once a year.   But mostly another seat is good for one of them in training.
