400 lbs. OSHA 50lbs per person, 8 people around stretcher max...=400 lbs. It's based on max number people you can get around the stretcher each lifting 50lbs. 

now, I recommend petroleum jelly for the arms of the patient. Also, if 4 feet tall, 400 lbs...probably quite large circumpherence. 6'2" 400 lbs, not at large.
