Hardest one is realizing what you think you know, that you don't know, and happen to be in line to suffer the consequences.  Skills are one thing, but judgement is the key.  To develop judgement one first needs to recognize the need, then identify a path to constantly gain and refine it.  Definitely the key staying alive.
