Message Should be an easy search. Just look for any HAA crash that the pilot survived. Could be two hours into shift and 3 things always get blamed. Fatigue, distraction and the good old standby phantom wind resulting in LTE or settling with power/ vortex ring/ recycled disturbed air molecules or whatever is the popular phrase this week.
Rarely does the pilot actually take responsibility and admit poor piloting.
