It takes a toll on the crew, MX and the AC. Morale and retention are both awful.  Every single talking suit in managment will preach SAFETY until they're blue in the face, but when it really comes down to it, safety takes a back seat to revenue.  Its only a matter of time before there's a fatal accident during a flight that never should have happened in the first place.

Hospital transports as many pax as possible via air knowing they'll never collect a single dime (demographics. Sorry, its just a fact) or medicare/medicaid/insurance won't cover the full amount.  Hospital runs millions of dollars in the red, with helicopters the main LOSS LEADER (look it up).  

Quarterly, they go to Uncle Sam with hat in hand, showing how their transport department is running in the red, thus using it as a write-off on the taxes. 

You think its a good thing to fly an EC135/EC145 160 hrs/month, 30+ legs per day,  hundreds of start cycles, takeoffs/landings and sujected to WX 24/7?

Been flying since 1968. Since I left the service, it don't make any difference to me why, who or what I'm flying. I get paid the same to "save lives" in EMS as I do to give rides at the fair, or the canyon or the beach. It ain't my fault some goober drove his pickup into a wall or ran over a kid. I don't save lives, the medics do. I just drive the bus.

Those flights that don't truly require HEMS are the ones that pay the bills for those for whom the HEMS is truly a benefit. Most of those trauma/emergent medical cases are no-pays or pay a very small percentage of actual costs.

The "American health care system" is well and truly fecked by being overly dependent on 'health insurance', primarily paid for indirectly through employer provided benefits. Even if you have 'cafeteria plans' available they are provided with advantage to those responsible, which are not you or yours.

As I understand it, your employers group demographics determine the plan structure and benefits available to you. Your actual benefit is very limited by the fact that YOU do not directly select options to maximise utility- you choose from the 'menu of limited options' available through your employer and it's contracted insurers. My opinion is that pilots, being a group of selected health risks by the professional requirement to meet pilot physicals should not be lumped in with the broader general demographics.

fatal accidents have already occurred during transport of a hang nail.  All this HAA stuff started in the early '70s as the military MAST program which proved to be viable, but not really a military mission.   The evolution of HAA has a miserable record provided by ignorant management on both the medical and aviation side.  Wish I could provide a silver bullet but the only I can think about ref: the subject is "slow down, think, figure how to do things properly".  Ain't gonna happen.
