In most cases they are paying passengers nothing more. Their input on the aviation side of the house means nothing. Think about it, when you fly commercial do you have any say in the operation or do you have any say on the treatment of the patient? Everyone needs to stay in their lane and perform the job assigned to them.

They are considered crew members. Hence why then undergo training every year at the direction of the FAA

Ask your "crewmember" if the tail is clear left.  They say yes. You turn your tail and hit something.  You will find out who the "crewmembers" are on that aircraft.

They are paying customers.

According to FAR 1.1 "Flightcrew member means a pilot, flight engineer, or flight navigator
assigned to duty in an aircraft during flight time."

If theyvwere just passengers, the. you'd be HAA without them, and it is clearly stated you are not. Your are only HAA when they are aboard (since you NEVER fly a patient without them yourself).

It don't matter what they are !  If their in your aircraft you are responsible for them .  Think and act accordingly.  Sort it out when you get back to base.

Ask the FAA for their interpretation. When the nurse and or medic are on board it is considered a Part 135 flight with or without the patient. FAA takes a dim view on calling the empty leg a Part 91 flight. In fact the only time you would call Part 91 is a maintenance flight, ferry flight, or a repositioning flight with only the pilot or maybe mech on board. Some people in this thread have a lot to learn.
