So the helicopter was hovering and suddenly pitched forward and flew sideways briefly and crashed. What would be the first reason you would think of to cause this? |

since the 130 does not have hydraulic pedals, I assume they mean swash plate instead of fenestron.
The hydraulic connections of each Fenestron servo remained intact; no leaks were noted to the connected servos. Both hydraulic reservoirs were near empty and the main hose from the reservoir was damaged with thermal damage from the post-impact fire. The forward driven hydraulic pump belt was partially consumed by the post-impact fire and no longer attached to the pump. |

I'm not at all familar with an EC-130, but I was actually thinking more along the lines of a servo slide valve seizure or servo malfunction of some sort. |

I remember when the TH-67's (206B3) at Rucker(Novosel) were having cyclic hydraulic anomalies.
One IP at Cairns lost an arm (they were taxiing at the time and aircraft rolled and he said he fought the controls but couldn't correct in time) and there were at least 2 more occurrences I personally heard about (another at Cairns and one at Shell as Shell had a few since they were divesting the 58A/C's).
Also, how about this? Any kind of Fiji NTSB report for it? Pretty significant noseover.

Slide Valve Seizure / Actuator Malfunction (Bell). More or less the same thing. Something, usually FOD of some kind in the hydraulic fluid, jams the port open and fluid continues to flow, pushing the servo/actuator to full travel. As I recall, it's like 400 lbs of pressure in the B407. Unless you can curl 400 lbs, you're not stopping it. All you can do it get the hydraulics off ASAP!
I had a slide valve seizure in an AS350 once. It was slow moving, but nothing I could do was stopping the cyclic from slowly moving right and forward. Hydraulics off and landed ASAP. Explained what had happened and the engineers drained and flushed the entire system, then replaced the fluid and filter. I flew that ship for another year and it never repeated. |

It was an ignorant AMC decision like so many others they make. Hospital wasn't involved this was a community based program. |

Slumping over the controls would cause these motions. Let's see what the cockpit camera shows. Why where they in that field to begin with if they wernt on scene? according to flight track the pilot made drastic turn and went to that field. Maybe he knew he wasn't gonna make it, went to land, and didnt quite get there and slumped over. Just like the guy in the gulf recently who did the same. |

I wouldn't put it past the FBI to inform NTSB they can't access or publish vaccine records regardless if he "slumped over". |
