Hyrdaulic Actuator Malfunction is the most probable cause. I had one once in a lateral quadrant with a student. It took both of us pushing the opposite way, the student was even using his leg to push, just to keep it from rolling uncontrollably. When I turned the Hydraulic Switch off we almost barrel rolled the opposie direction. |

Bird Strick, Incapacitating the pilot or flight controls. Does the operator have GEO fencing for bird strick midigation like PHI, last flight track shows around 1500 feet and in an area that would be a hot bed for bird migration, Believe recommended flight in that area is above 2000 feet for some operations. To early to really speculat. Very sad situation |

or sanctuaries as indicated on the map. The procedure for some operators is to fly no more than 100 knots over the marsh which is claimed to reduced the damage that might occur if a bird strikes the helicopter but in reality is just to gain minutes to bill the customers. |

A bird strike would probably leave significant evidence: blood, guts, feces, feathers, and often the actual bird. I knew a guy who had the bird come into his lap that way. He said the bird was pretty unhappy with the helicopter ride.
I hit one at 2000' and climbing, at cruise speed. It was in the mob trailing a shrimp boat culling their catch. The carcass didn't come into the cabin through punched out TwinStar nose cone but a lot of other 'stuff' did. |

1 Helo low level. Buzzard hit main rotor. Very little damage.
2 Jet low level. Unknown bird medium sized hit front wind screen. Could barely see out front. Little damage.
3 Jet about 2000 AGL. Unknown medium sized bird hit horizontal tail AT NIGHT. Moderate damage on leading edge. |

IIMC Uncontrolled Flight Into Terrain (UFIT) was wx a factor
A fellow PHI pilot had the back end of his 407 get cut off by its own tail rotor. Although the resulting nose-down was extreme, he was able to get it level before landing on the water. In a 206 that would probably be a fatal.
The RLC pilot was fairly young, so while a physiological event can't be ruled out, it's not highly probable.
Nobody knows. Let's hope it wasn't a catastrophic failure of some kind. Bird-strike seems like it would be high on the list of "maybe's." We'll see. |

In a 206 that would probably be a fatal.
There was a Era 206B in the GOM in the 80's that lost everything just behind the horizontal stab and the pilot was able to get it to the water right side up. I don't think the cabin even got wet. |

incapacitation? Weather event like microburst - been in one, along for the ride,yes we were lucky it didn' take us to the ground. |
