I was reading the new medical requirements for part 135 starting January 2024.  
Not a big deal for most.

First class medical required after 60th birthday renewable yearly.  I'm guessing they're looking for the EKG here. Revert to second class OK.

First class medical required after 65th birthday renewable every six months. In other words you have to maintain a first class after age 65.

And after your 65th birthday you have to have a yearly evaluation by a physician for dementia.   Not sure how that will work.

So not a lot different than part 121 other than you can keep flying after your 65th birthday if certain medical conditions are met.

My company has used the same medical criteria for years, except for the dementia screen. Trying to make a career as a commercial helicopter pilot is already proof of dementia...

requirement after 65 it is still less thorough than the over 40 military Class 2.

I'm in better shape then all the other pilots at the base at 65. 40 year fat guys should get their class 1.
