Hopefully nobody got squashed. Hook releases are fairly uncommon but could have been from where it was installed incorrectly. Twitchy fingers are fairly common. |

especially if its ur livelihood. Thankfully I did US both in military and civil and never punched one off but I did come close with one that I couldn't get over 35 knots while IMC. Unbeknownst there was a board sticking out one side of the wireline basket - went back to the wellhead where I picked it up and the handler fixed it quickly and I was on my way, but, had to divert for fuel before getting to its destination. About 2600lbs with a 212 with -B engines. |

Value of aircraft and safety of others = priceless
Load was dropped for a reason in a place where it could do no damage or injury. better there than on someone while trying to be a get-er-done hero |

In nearly 25,000 hours of Heavy Lift work I was lucky enough to never punch one off unintentionally, however I did have a few hook and rigging failures. No one hurt.
I agree, don't be to quick to judge without knowing the facts.....
