Back then, the medical crewmembers were considered Part 91 crewmembers and not Part 135 passengers. They lobbied for the change in "status" and got it in the 2014 revamp of HAA rules under Part 135. If they didn't want to be left behind, they shouldn't have demanded they become Part 135 passengers the certificate holder couldn't operate Part 91 with.

It's on them, not you or the certificate holder!
