Conducting a literature review for accidents/incidents involving fatigue due to long duty days or not getting minimum required rest. If you can remember an accident or incident that the FAA or NTSB cites as a direct or contributing factor exceess of duty time under Part 91, or fatigue due to long duty days, particulary past 14 hours, please provide this information here. Simply case and date (even if approximate) to zero in on the actual reports would be appreciative.
