It is noteworthy that they crashed at an altitude that was *BELOW* the altitude of the airport they just departed, and nearly headed back toward it.  They never did get a positive rate of climb going.  We can "Why?" it to death, but in the end, like many accidents, we just don't - and never will - know why.

I worked for PHI at the time and can tell you that it was a VERY disturbing event for the company, maybe even more so than some other fatals we had.  Nobody liked to talk about this one.  Every time it was brought up, the subject was squelched or changed.  I heard rumors...but you know how that goes.  It's clear that there was stuff about the crash that the company kept quiet.
