I can't tell you what the price difference is, because every doctor is free to set his own price. 

I will tell you, that the FAA medical is not what you want for an annual physical exam!  Remember that the doctor you are seeing is at the moment of your examination an agent of the FAA.  His responsibilty is to them and not to you.  If he lets things slide that would normally be a non issue for a family doc, he is jeopardizing his medical ticket with the FAA.  Just the same as a pilot cheating on a written or on a checkride.  Now, that's not to say that every flight surgeon is a D**K and is out to ground you, but you have to remember where their loyalty lies.  If you find an issue with an outside doc, often times that issue can be resolved BEFORE you go to the flight surgeon and might not cause a disruption in your flying.  Also, the FAA medical does not require any blood draws and lab work, so from a health standpoint you are missing out on much of what you should be getting from an annual physical.

As far as the EKG goes, it is sent directly to OKC medical review as soon as you are hooked up.  He has no opportunity to review it before sending, and if there is ANY question about it, he will probably deny your cert. until the medical division has a chance to review it. 

I was always told to never get more of an FAA physical than is required.  Of course these are just my opinions!
