a helicopter had an engine failure in the LA area.  The closest place to set it down was a small area in a parking lot but in order to put the helicopter there the pilot had to clear some power lines, you can imagine what the 500C looked like after what amounted to a 50' hovering autorotation.  The resulting injuries cost the pilot his medical; the cause was found to be an improperly serviced fuel control.

In order to collect more than his medical bills the pilot had to take the insurance company to court.  The insurance company claimed that the pilot did not have the hours he claimed, the pilot did not have a log book to prove anything other than time for ratings.  The pilot lost.
