just realize the risk in single pilot and engine night HAA at such hours will eventually catch up with quite a few pilots, and there's not a lot that can be done about it.

All the little play-time add-on solutions, risk assessments, call the Ops Center for release, and so on do nothing other than convolute logic and make decisions more complicated and obscure.  

What is needed is far more than anyone is willing to pay.

I suggest the industry start simplifying and reducing the use of helicopters in patient transport.

Nothing else will reduce accidents.
