There aren't any cases of loss of tail rotor authority if the aircraft is'nt operated over allowable performance gross weight and is operated within wind limitations. Some aircraft have a smaller margin of safety but all are safe if the pilot operates within the limitations. Buttom line, much like other aspects of safety, the pilot is generally going to be a big part of the problem in crashes.
