active and other standby on box 2, flip/flop as required. Use box 1 for ATC obviously. The hospital has absolutely zero control over your command pilot obligations. I suggest you convene an informal chat with them via your management. Again, remember, they are NOT in charge of you, you do your job as required and if push come to shove start reporting to your company and NASA ASRS and the FAA. Is what being a PIC is all about.
